Nurse Case Management: Disability and Rehabilitation Case Management Services

Our Nurse Case Management services are designed to ensure continuity of care. Our mission is to provide high quality rehabilitation services, while effectively reducing costs associated with injuries. We partner with the employer, medical providers and the injured employee to return the individual to a productive lifestyle and suitable employment as quickly as possible.
ORCI will work closely with the all involved parties on return to work goals. Return to work planning is critical to any program. Certified Rehabilitation Counselors begin return to work planning upon receipt of a case. A Return to Work Plan is developed for each case to assign time oriented, appropriate, and obtainable goals for the injured employee. The Return to Work Plan includes the medical treatment needed, the time frame for this treatment and estimated return to work dates for transitional and/or full duty employment.
Our case management services include telephone and field case management to include attending appointments with treating and consulting physicians. ORCI will provide all parities with a medical update within 24 hours of any appointment. ORCI will also schedule Independent Medical Exams (IME’s), Functional Capacity Evaluations (FCE”s) and other appointments as needed.
We will maintain in person, telephone and written contact with all involved parties to inform them of the progress on the case. ORCI believes in a customized approach and will work directly with the customer on each case to determine the level of services required.
Home Modifications
As an additional service, ORCI will evaluate homes and recommend modifications to allow for independence and accessibility. Recommendations may include construction and equipment modifications to kitchens, bathrooms, bedrooms, living rooms, entrances, exits and doorways.
